Most pregnancy calculators online take it from the day of your last af.  Whilst the calculator on MM's uses your last af date to work it out it takes it from 2 wks later, roughly when most people would ovulate and gives you the date from then.  The hospital measurement is done slightly differenlty as in they expect on average that most people with have a gestational sac of so large by such a date.  The important thing to remember is that none of us are average so you can measure bigger or smaller and that will still be normal for you at that gestation.
