Like many have already said its good to write it all down. Your feelings, what sets things off etc. Then you can go through these one by one and tackle them rather than trying to do it all at the same time.
I had PND after having my twin girls and it took me over 2 months to admit what was wrong. I did have other problems aswell at the time but it all mounts up. We are all good at convincing ourselfs we are ok even when we know we're not.
You have took the hardest step by admitting there is something wrong. But please do not think this is your fault as its far from it. Most ladies on here have either had PND, depression or both...
There is a buddy scheme you can do which is one to one.
I would recommend going to the docs and explaining everything to them, even if you break down your not the first they have seen and you won't be the last. I know i did when i went to see mine.
This is the next hard step doing something but you have done the first so go for it. When you feel better you wil be so glad you did and you won't be afraid to admit it either.
